Annual Registration for all using the Food Pantry

Each October we are required to reverify the names and addresses of clients utilizing the food pantry. At your first visit after October 1st, you must provide an ID and Proof of Address for anyone in household 18 years of age and older. For children under 18 you must simply verbally verify the names and dates of birth.

For an ID please bring a driver’s license, Wisconsin ID card, insurance card, school paperwork, birth certificate, passport, custody papers, rent receipt, deed or mortgage. A social security card can be used but we cannot ask for it. The person picking up food must show an ID with each visit to the pantry.

Proof of address for those 18 and older could be the lease, car registration, current bill, current first-class mail with address, letter or communication from a school. Any original, photocopy or electronic record on your phone would be acceptable.
